I received exactly the same letter as you described. I've received a handful of calls to me cell phone that showed up "Unknown Caller", so I DID NOT ANSWER. We routinely receive phone calls to our landline that are "junk". We only have the landline because it was free for a year, and then $10 a month in a bundle with our cable TV and high speed internet. We VERY RARELY use it, except that it comes in handy when you're completing some type of registration or whatever and your phone number is a required field. We use our landline number. That provides a legit phone # to meet the requirements, AND prevents multiple nuisance junk calls on our cell phones.

My letter was verbatim the same as multiple others listed here. Letter was dated Jan 20, 2016.

Interesting to note, we've had Anthem Blue Cross health care insurance for 30 years. PLEASE DON'T MISUNDERSTAND ME. I AM NOT STATING THAT THIS LETTER IS RELATED TO ANTHEM BLUE CROSS IN ANY WAY. I am merely pointing out the formatting. Perhaps this was done on purpose in an effort to "reassure" unsuspecting folks and lull them into calling the phone number listed in the letter, and potentially providing these scammers with vital health care or financial information. Healthcare Identity theft is becoming as real of a problem
as financial identity theft. For that reason, I'm going to provide quite detailed information about the letter I received, so I apologize in advance for the length and detail of my post. I hope this helps somehow.

Now....back to Anthem Blue Cross...The entire format/layout of the top 1/3 of the page is nearly identical in appearance, font, spacing, etc to benefit and coverage notices sent out by the insurance company. As a matter of fact, I didn't immediately open the letter for that very reason. I thought it was a benefit statement regarding a recent Dr visit and put it with the bills to be paid when I received a statement from the Dr's office. BOTH of the addresses (my address, as well as the return address) were absolutely identical in format, included a bolded font line just under the return address with the following wording:
"Forwarding Service Requested" Just below those words were bolded, all caps "IMPORTANT MEMBERSHIP NOTICE".
Due to HIPAA rules regarding privacy of healthcare related information, it is to be expected there would be no information identifying WHO the letter was from on the outside of the envelope or through the window of the envelope. The only information that would be visible would be my name and address, the return address, and the two lines of text I just typed. That's so similar to an insurance statement that it's mind boggling.
There are also a couple of bar-codes placed vertically in the margins on both sides of the bottom third of the letter I received. The bar code on the R side is about 1.5" long, and is shorter than the one on the L, which is about 3.5' long. There's a third bar-code that's formatted slightly differently located just above my name and address at the top of the page. It's about 3" in length.
The letter I received includes an 8 digit "Member Number:######## (I'm not including the actual number in case it would provide any identifying info about ME!). The "Member Number" is printed just above the date (January 20, 2016), on the top 1/3 of the page...in an area that's usually reserved for information indicating WHO the letter is TO, FROM, date of letter, etc.
In addition to the bar-codes and the "Membership Number"-
There's a series of very small characters printed just beneath my name & address...specifically just below my town. ####### - X### - #### (I typed the symbol # for each number, included hyphens as they appear, and typed the "X" to indicate where it appears in the series of 15 characters). There is also a tiny square QR code immediately preceding the set of 15 characters, but located slightly towards the L, not directly under my name and address.

There's another tiny QR code in the very bottom L corner of the page, as well as three separate lines of alpha-numeric characters, all in the larger, normal sized font. The 3 lines of characters are printed immediately above the QR code at the bottom L corner of the page. I'll use the # sign again to indicate characters that are numeric.
The first line includes one uppercase L, followed by 4 numbers, a space, and 4 more numbers. L#### #### There IS a space where indicated.
The second line includes one uppercase X, followed by 4 numbers, without any spaces X###
The third line contains all uppercase letters CB-COMP

I hope this information is useful in some way to prevent innocent victims from being preyed upon by these low life scammers. Its a crying shame that they choose to focus their obvious talents on scamming people instead of using them to benefit society in some way!

I am sick and tired of all these scam calls I receive. I have already blocked 30 of these calls, and they just change their numbers and keep calling. I get around 5 scam calls everyday. Called them and automated message said, "Due to higher than normal call volume, response time may take awhile"(etc). Someone answered in less than 30 seconds. I asked the woman who answered what service they provide. I was told that "Hot-Line" offers credit card protection service. I told them, "No thanks," and hung up.

This MAY be legit. But, PROBABLY NOT. After all, I have had 'free" account benefit life insurance from B.of A. for about 20 years. But that would end, if I end the account. This MIGHT be a company that was providing fraud insurance coverage paid by a credit card company in the past and one of the two companies is now clearing out old records. My dad, who received the letter, says he DID have some similar benefit included with some credit card he signed up for in the early 90's, but couldn't remember anything about it.

It's MORE LIKELY that some credit company is dumping their 20-25 year old records and either sold the non-private info to some marketing company or had the records stolen stolen from their trash or processing room.

I got the same letter described in the comments here. The letter was addressed to my maiden name that I haven't used in 14 years and to my mother's address. I called the number 855-444-8283, and spoke with a woman named Seline (heavy accent). She told me that the "membership" was associated with my credit card and asked me for name and address. I told her I wouldn't give that information until she could tell me how this membership was initiated and more details about her employer or the organization affiliated with "Hot-Line membership". She refused to give me that information and I told her to remove my name from any of their mailings and client lists, and told her I would report this if they contacted me again. This is a scam.
